Arrhythmogenicity of the Coronary Sinus

چکیده
The coronary sinus (CS) is the cardiac venous system that begins at its ostium in the right atrium and ends at the origin of the great cardiac vein.1,2,3 The major tributaries of the CS include the great cardiac vein (anterior cardiac vein), the left obtuse marginal vein, the posterior (or inferior) left ventricular vein, the middle cardiac vein, and the right coronary vein. In addition, atrial veins and, notably, the vein of Marshall (or oblique left atrial vein) also enter the coronary sinus.1,2,3 From the perspective of electrophysiologists, the CS represents an anatomical structure of particular interest. First, it provides access to epicardial atrioventricular pathways.4,5 and arrhythmogenic foci of both atrial6 and ventricular arrhythmia.7,8 Second, it represents by itself a potential source of atrial arrhythmia.9,10,11,12,13,14 The arrhythmogenic potential of the thoracic veins in general has been recognised since the 1970s.15,16 Atrial arrhythmias can originate in the pulmonary veins,17 the superior vena cava,18 and the CS. Indeed, biatrial flutter,19,20 left atrial tachycardia,9,10 and atrial fibrillation12,14 involving the distal CS have been well described. There is now evidence that the CS apart from participating in arrhythmia circuits, such as in the slow-slow form of atrioventricular nodal reentrant tachycardia21 and atrioventricular reentrant tachycardia due to accessory pathways,22,23,24,25,26 may itself be a source of apparently atrial arrhythmia. In patients with paroxysmal atrial fibrillation apparently originating from the left superior or inferior pulmonary vein, detailed epicardial mapping through the distal coronary sinus might identify epicardial location of the arrhythmogenic focus.6 Therefore, the search for foci of abnormal automaticity within the CS should be part of the electrophysiologic evaluation of left atrial arrhythmias.13

High Atrial Defibrillation Threshold With Internal Cardioversion Indicates Arrhythmogenicity of Superior Vena Cava in Non-Long-Standing Persistent Atrial Fibrillation.
BACKGROUND The purpose of this study was to clarify the relation between atrial defibrillation threshold (ADFT) for internal cardioversion (IC) and arrhythmogenicity of the superior vena cava (SVC). METHODS AND RESULTS A total of 159 consecutive patients (139 male, age 59.9±10.3 years) who underwent radiofrequency catheter ablation of atrial fibrillation (AF) were assessed. IC was performed i...
متن کاملBicuspid Aortic Valve: An Unusual Cause of Aneurysm of Left Coronary Sinus of Valsalva
Bicuspid aortic valve is traditionally considered an innocuous congenital anomaly. Due to a better and widespread availability of non-invasive imaging techniques, it has come to the fore that 30% of these cases develop complications, viz., valve abnormality (aortic regurgitation and stenosis), and aneurysm of aortic root and ascending aorta. Sinus of Valsalva aneurysm is an uncommon complicatio...
متن کاملPseudo Cor Triatriatum Sinistrum due to Dilated Coronary Sinus in Association with Supramitral Ring
We herein describe a 34-year-old woman with a persistent left superior vena cava (PLSVC) impinging on the posterior wall of the left atrium, subdivided by a membrane at the distal part of the impingement. In this patient, a much dilated coronary sinus due to the PLSVC resulted in a membrane-like structure in the posterior wall of the left atrium with the appearance of the cor triatriatum in tra...
متن کاملA New Coronary Retroinfusion Technique in theRat Infarct Model: Transjugular Cardiac Vein Catheterization
Cell delivery via the retrograde coronary route boasts less vessel embolism, myocardial injury, and arrhythmogenicity when compared with those via antegrade coronary administration or myocardial injection. However, conventional insertion into the coronary sinus and consequent bleeding complication prevent its application in small animals. To overcome the complication of bleeding, we described a...
متن کاملپارگی آنوریسم سینوس والسالوا چپ به شریان پولمونر اصلی: یک مورد بسیار نادر
Sinus of valsalva aneurysm, is a rare congenital defect and the diagnosis is usually made after it ruptures. The anomaly often involves the right coronary sinus and is less frequent in the noncoronary sinus. The rupture of the left sinus of valsalva into the pulmonary artery is extremely rare. So far, few cases with ruptured left sinus of valsalva have been reported. We are reporting a ruptur...
